There are times after an incredibly cold shower, or cold pool time that my nipples retract significantly, and my chest almost looks normal.
So I was wondering what happens on the table for surgery, when the incision is made and glandular tissue retracts?  Or if the simple action of having your shirt off, going in to surgery in a cool room...how would the surgeon best know how much gland to excise?  I realize they mark you up before hand, and evaluate what will be removed, but wouldn't it be harder to extract tissue, when your nipples are stimulated, and glandular tissue is compacted and retracted?

Not feeding my ego here by bumping this up, I just think its an important question, that may have been pushed too early to page two for the doctors occasionally lurking here to see.

Broadsidebrother is right.  You can see the gland tissue when you cut through the skin.  Whether or not the nipple/areola is "erect" in surgery is irrelevant.

My question is after my gland was taken out 5 weeks ago i though the nipple wouldn't still puff out, but the top part is still puffing out some. Why is that. The doc took the gland out. I hope its the scar tissue doing it. When its cold it looks fine. Its still a lil sore so i hope its just still swollen.

At 5 weeks postop, you're probably still swollen.  I would give it more time.  Be sure to follow up with your surgeon and discuss this with him.

I go on tuesday for a fallow up. I hope its swelling, everything looks good except the top part, its puff on the same place for both nipples, just the top. The whole thing itself looks alot better, but would be perfect if the top part would flatten out.
