I have puffy nipples. Soon I will decide and have an operation. The problem is that some plastic surgeons I have seen so far said that I needed both liposuction and excision whereas some PS said that liposuction alone would be enough because my gyno is mainly fat.

I am really confused now and dont know which way to go? As far as I know puffy nipples are caused by gland but not the fat and the only way to remove the gland is by excision. Do any of you guys had puffy nipples before and then flatten them with only liposuction but not excision?

Please share your experiences/comments and help me decide..Many thanks in advance

I also have puffy nipples. I have chest muscle but the nipples stick out. My doctor said that I do not have gynecomastia or any gland tissue.

I guess it is fat behind the nipples. I will do a lot of cardio and try to BURN it off.

BURN IT UP!!!!!!!!!!!!

fat behind the nipples??? now I have one more diagnosis to choose from...sorry for the bad news but I lost like 25 pounds some time ago but it did not work!! fat does not cause puffyness and no matter how much you burn, they just stay there  >:(

I had surgery I think about 6 years ago (Lipo only) I wasnt satisfied but it did help.  I told the Doctor that my nipples were still puffy and they didnt get all of it out.  He said yes they got it all and they would go away when I fully healed.  I was about 25% bf then, and they did go down with the surgery alot but still are puffy.  Most of the gland i feel is gone I beleive but my nipples are still pretty puffy.  Over the last year I have started back exercising and weight training and I have noticed a decrease.  I was 6'-0" and 235lb and now Im around 215.  Also I was diagnosed with Gyno and I remember having it in my early teans.  I didnt do anything about it until I was about 30 years old.  After I did go it was like it was no big deal and just a part of life.  I really dont want to go back so I bought 'diet aid of questionable value' which is a natural herbal pill taken 3 times a day.  I bought like 5 months worth but I have actually skipped a week at a time.  I kinda think it has helped a lil but hard to say.  I have like 2.5 months to go, so im going to stick to it religously the rest of the way.
Another thing I have noticed is before my lipo my nipples were puffy but more solid puffy if that makes sense.  Now there actually super soft.  I can actually almost take em and fold them around kinda.  Its more like air or almost like empty ness if you know what I mean.  Could this be just fat or Gland tissue still.  Anyway I just got back to trying to figure out what to do in the next 6 months with it.  It really dont bother me like it use too but now since im 36 and seeing results with my working out and cardio I would like to flatten them out for this summer.  I really am not wanting to do surgery again because taken more out could really screw me up after seeing some pictures ive seen.  My chest is a little different with the nipples now from last lipo but not too noticeable...  Also if I push my nipples in I can feel muscle behind them and not the disc gland like back then.  I just would like to get the aortas flattened.  I wore a garment after surgery like they said to do but I guess it would be too late in the game to wear some type of a compression shirt of some kind now???  Anyway I have my doubts on this gynyxin or what ever.  I wonder if its just a void in there or maybe water.  I also have looked into the burn cremes that supposely burn fat but i have my doubts on that too.  ANybody going through the same shit I am raise your hand...???  I can post some pics later if I need to.  I have read about getting out of shower and being cold, and have noticed when im that way my chest looks 200% better...

Both fat and gland together push the nipples out. Since you had only lipo you got one of them removed. But the other one is still in there and keeps making them look -less than before- but still puffy.

Hello,
Im new to the site. I have had puffy nips for as long as I can remember. I did allot of cardio as what I was told and found that the condition actually got worse  :-\ because all the skin around the nip got tight but the nip. It was a terrrrrrrrible feeling. I then was told by multi guys that if I got big IE muscles I would loose the look. I did find that to work. But my nips stayed big. I finally got the balls to talk to a plastic surgeon and went for it. I figure it couldn't get any worse???????? Well 6 days after surgery I'm a lil bummed cause they look like they are coming back. The nip got real puffy the last few days and I don't know why????? My wife and the surgeon says its swelling and in a few weeks will go down and I will love the results....I say go for the surgery my friend, no one understands what its like to have the condition. You feel different and its not right. I hope that my swelling goes down and they look manly cause puff nips sucks. Go for it!

"Puffy Nipples" is a common public term for gynecomastia.

The problem is the the term puffy nipples is a phrase that mean so many different things to so many different people.  Words just do not convey the actual problem very well - images do a little better, but still are not the same as an actual in office evaluation with your doctor.  This Gynecomastia Picture Gallery of Puffy Nipples begins to show some of the various problems patients have called "puffy."

Putting up pictures (using standard views before and after surgery) is one way to discuss what the problem was before surgery and what has happened.   Options depend on what is really going on.

Let us try to look what I mean by the problem of the words only descriptions.  "Large nips", "puffy nipples," "puffed nipples,"  and "puffy nips" are a common terms many give to a problem that extends to the region about the areola.   The nipple is actually the central raised structure inside the pigmented areola.

"Puffed nipples" can be a problem for some that involves long nipples above the areola where nipple reduction alone helps.

"Puffy Nipps" can be a problem behind the areola that can take many forms.  The deformity is usually a varying combination of fat and gland.  Check out these series of drawings I did to demonstrate the Anatomy of Puffy Nipples.

The gland can be firm or soft, spread through fat, or be a condensed mass.  There is a thin muscle under the areola skin that can flatten tissue when stimulated. Unfortunately it is impractical to keep stimulating these muscles.

In many of these patients with "puffy nipples," there was no firm tissue under the areola, just fat and soft gland.  Here is one example of puffy nipples in a muscular male.  Here is another example of   puffed nipples.  Here is another patient with puffy nipple gynecomastia.

"Puffy Nipples" can also be a combination of gynecomastia and big nipples.

I prefer my Dynamic Technique that adapts to the problem found during surgery to minimize such issues as residual deformity after surgery.

Hope this helps,
