Portland/Seattle surgeons
Any suggestions for good surgeons in the Pacific NW?  Also approximate costs?

 I'm looking for one as well...seems not a lot of people have had this surgery around here...on these forums anyway.

Dr. Keith Paige @ Virginia Mason in seattle, WA
He's done quite a few and was recomended by my endo.

His waiting list is about 10 months to schedule surgery though....
