Question about reducing nipple size

I have gynecomastia and my nipples look like pepperonis. My question is when you have surgery do they reduce the nipple size when they take out the gland or is that an additional cost to the surgery?

It will most probably be an additional cost because It will take extra surgery time.
It is a common prosedure for an expericened gyno surgeon and the result is going to be worth the extra cost. (had It myself)
