How good is the traditional old way of using a horizontal scar across the chest and underarms for cases with excess skin and sagging

There is going to be prominent scaring but I was told it is a good trade off as lot of skin can be removed with this process

What are the advantages and disadvantages of this technique? How different is this from a free nipple graft.

Skin excess in patients with gynecomastia is a complex subject. There are a number of causes and tremendous variability as to the extent of the problem. Each case requires an individualized approach that balances the risks and  resultant cosmetic deformities as a result of the intervention.Nothing gets rid of as much skin as a big resection of skin however nothing results in as much scarring that is visably noticeable as a big skin excision.The goal is to make the patient look better not different.With respect to your situation some photos would be helpful.

Depending on the distribution of skin and tissue excess, there are a number of ways to approach skin removal, and one very effective way is with a horizontal scar, which in the best case can be hidden in the natural line of the infra-mammary fold.  Something must be done to relocate the nipple, and this can be done with a free nipple graft in very severe cases, and possibly with a small "inferior pedicle" to which the nipple remains attached in less severe cases.  Photos would be helpful, and be sure to consult with a surgeon who is comfortable with many different approaches to gynecomastia correction.
