You are holding a good amount of fat from what I can see. So it's hard to tell if it's glandular, fat, or both. You would have to get it inspected in person by a plastic surgeon in order to get a real answer. If you had lower body fat then it might be easier to tell, but you could very well just be holding excess fat.

I agree with the previous poster that you have some extra padding; just look at your flank areas.  I'm sure you will feel happier by losing 15-20 lbs by exercising and watching your diet.

Yes,  and there are over a hundred different drugs that "Might" be a problem.  The degree of risk that it is a drug side effect is small, but it is possible.
