Background Info - I first noticed puffiness around my nipple around 8 months ago, prior to that I had put weight on, muscle and fat but never noticed it before.

So, I went to the doctors to check If I had Gynecomastia. He said my chest looked puffy, asked me if I took steroids which I don't and poked around my chest. He then said I have nothing to worry about and it was fat. BUT, I can still feel gland like shapes under the nipple if I lie down.

Should I go and ask again? I'm becoming really quite self-conscious wearing t-shirts as my nipples are visible. I'm not too sure why I would have developed this in the first place.

You appear to have puffy nipples, which is always produced by gland under the areolas -- fat alone just doesn't do this.  the gland may feel soft -- so feeling for "gland" won't make any difference.

Also, just like an iceberg, there is more tissue under the skin that you may realize.  Treating cases such as yourself is tricky -- best to do your research and go to a gyne expert in your area.

You have puffy nipples, which is a mild form of gynecomastia, and as Dr. Jacobs indicated, this can be trickier to correct that more moderate to severe forms, since there's not much room for error. I see many of these sorts of cases related to puberty and also to use of anabolic supplements, since the patients are often lean with mostly glandular tissue under the nipple/areola.  If you haven't already found a surgeon, you should be careful to find someone who is an expert and can get you a good result. When your Dr. said you only have fat and nothing to worry about, I believe he meant that you do not have a health issue that is something to be concerned about. 
If you have a concern about the appearance of your chest, you should seek the advice of a plastic surgeon with expertise in gynecomastia surgery. 
All boys and men have a small mass of fibrous breast tissue immediately below the areolas. This small mass of breast tissue does not become apparent externally unless it grows in size to a point where the nipple-areola complexes begin to bulge outward, hence the term ‘puffy nipples’.
 
Treatment consists of excision of the excess breast tissue using one or more of a number of techniques: direct excision via an incision along the inferior border of the areola, ‘pull-through’ excision through two or more mini-incisions at the areolar border, and arthroscopy shaver excision via a remote mini-incision (ideally concealed in hair-bearing skin in the underarm area). As there is no fat excess in patients with pure glandular gynecomastia, no liposuction is required.
 
Care must be taken on the part of the surgeon to perform a precise excision of the excess breast tissue, and to preserve a thin cuff of breast tissue on the posterior surface of the areolas(s). Over-resection will result in areolar retraction and concavity as well as contour irregularity in the area immediately surrounding the nipple-areola complexes.
