Tingling

I occasionally get a sensation or tingling in one or both of my nipples when urinating or having a bowel movement, is this a sign of gynecomatia or more a lymphatic issue? I have read that testicular cancer can also cause gynecomastia, I have no noticeable changes in size of the breast or mass behind the nipples

I have never heard of such an association. I really can't think of how that could happen.

I would agree with Dr. Schuster that this is not a complaint I've heard described in the past.  Alternatively, many men (not those with gynecomastia based on anecdotal experience) derive erotic pleasure from nipple stimulation during sexual activity, so some pathways exist that lead to a response in the perineum following stimulation of the nipples. I'm not aware of this being related to gynecomastia in any way. I advise patients that this connection is likely to be absent after glandular excision in a gynecomastia procedure, as this cuts the nerve branch that provides that sensitivity.
