Why does gyno tissue get bigger when hot? Also is it easier for surgeons to perform surgery with the nipples hot and puffy or cold?

There is a muscle in the areola which is active with stimulation.  When cold or stimulated it contracts to make it smaller and when warm it relaxes and the areola expands which allows the breast gland to protrude more.  This does not affect surgery in any way Good Luck.

Puffiness is always due to the presence of breast tissue under the areola itself -- not fat.  The areolar skin contains tiny muscle fibers within it -- that is why it become wrinkled when exposed to cold.  

When the nipple is exposed to cold or to physical/emotional stimulation, those muscle fibers tighten up, the areola shrinks in diameter and literally pushes the protruding breast tissue into the chest, thereby making it look like the gynecomastia has diminished.  It hasn't.  When the areola relaxes, the original puffiness returns.
