Can someone provide a list of questions to ask during a consultation?

For the benefit of everyone who will get consultation, I think it would be a good idea to prepare list of questions BEFORE meeting with the surgeon.

Such as could there be any extra fee that can come up after the surgery?  Someone has said that in the state of Illinois, the state law requires any tissue removed from a patient must be test for cancer meaning the patient will be charged for the processing of fatty tissue/gland.

Please provide as many questions as you can think of to ask the Surgeon.
