Sometimes (if not all the time) when I do some exercise like football training my nipples go back to 'normal' nipples instead of puffy nipples. Why is this?.

There is a thin Muscle just under the Nipple Areola that will Flatten Puffy Nipples when stimulated. When this structure relaxes, the underlying contour is again revealed.

Typically my patients say when it is warmer out, their puffy nipples are worse than when it is cold.  Some patients use cold water to make their puffy nipples appear less noticeable and many of my patients have a hard time with hot tubs for this very reason.
