please tell me if i actually need to go under the knife or i can work it out??  i been trying for years but didnt work but also scared of going under knife.....

You have gynecomastia.  Surgery is the only way to correct it.  No amount of working out will get rid of it.

I suggest that your first step be to loose a few pounds. That alone may give you a "good-enough" chest with which you'd be comfortable. But if your goal is a muscular, well-defined chest, then weight loss, lots of exercise and surgery is the route you need to take.

im still cofused... i have started my gym again.  im hoping to loose another ten pounds see if i can get a batter looking body....

You have gynecomastia.  And you are also a bit overweight. I have operated on men such as yourself at your present weight -- and then after surgery, perhaps inspired by having a better chest, those same men went on to work out, lose weight, etc -- and their chest got even better.

On the other hand, there is nothing lost in working out and losing weight.  But you will still have gyne and still need to have surgery in order to improve upon what remains.

There is no right or wrong approach -- either one works.  What is important is for you to assess what your immediate and long term goals will be.

I was wondering if my condition requires lipo or some other type of surgery? 

My long term goal is to have perfect body with 6 packs and perfect chest so i can wear those small tshirts..... :)

im 30 years oll 5,9 with 180 pounds.  Do you think 170 would be perfect for my body?

5'9 and 170? woah.

I'd think closer to 155-160.

Many guys want to have a body close to what they see in the muscle magazines.  Unfortunately, in order to achieve those looks, one must devote practically their entire lives to working out, following a very strict diet to get their body fat percent extremely low, and in many cases, take anabolic steroids to build that type of muscle.

For the average guy who has to go to work to support himself, that is an unattainable goal.  And taking steroids is never a good idea.

However, if you work out and get down to 170 or even lower, you should have a very nice physique -- but you may not have that bulging 6-pack.

And yes, when you lose the weight you will probably have residual gyne, which would require surgery.  and the surgery usually involves both lipo and direct gland excision.
