nipples still puffy after surgery

i had surgery in december for puffy/ potruding nipples. the surgery went pretty well and a lot of the puffiness/ potruding went way down. It looks way better but i think the surgeron might have left to much breast tissue. it is pretty minor but i still dont have the chest i want. So my issue is that the it is kinda minor with only a little bit of puffiness but i want the fat nipple look. If it is to minor then will my nipples cave in instead of going flat?
