Pseudo? No?  Surgery needed? 5'8 and 190 pounds

It is gynecomastia when ever you have male breast enlargement! As for surgery, that is a personal choice! Can you live with your breast? Do they bother you?. Do you have $3,000.00 or more to have the surgery?

There are some of us that have had them all our lives and don't have problems, have accepted the breast, so it is a personal choice.

It does appear that you have excess tissue on your chest.  I don't believe in the presence of "pseudogynecomastia" and have spelled it out in my thread:
