14 months ago i had a gyno surgery which satisfied me greatly first times. However i started to notice my right nipple is still swollen and a little bit sensitive compared the other one but not bad as before. My surgeon said that she left a little bit of gland tissue on each sides to prevent any deformation. Could the tissue is growing again ? (I had my blood tests and my hormonal levels are fine)

It is difficult to judge anything from the photo based on the distance it was taken. You should retake the photo from  8 to 10 feet away.

I would agree that the photo is not helpful.  It's possible that you may have some scar tissue that is responsible for the difference, and this can often respond to a corticosteroid (Kenalog) injection, if you doctor feels this is warranted.
