Nipple piercings are typically placed through the anatomic nipple. It is the areola muscle that, when stimulated can flatten "puffy nipples." You can see the various names for the male chest anatomy here. I have seen some very nasty deformities from piercings that patients have asked me to reconstruct.  Some deformities have surgical options, others just do not.

Thin muscles just under the areola can flatten the areola when stimulated.  I know of no practical way to keep that muscle stimulated all the time. I also have seen some severe mutilations people have caused trying to keep that muscle tightened.  

Gland and fat tissue behind the skin are the most common causes. Sometimes redundant skin can be a factor.
