Reason for swellings

doctors swellings appear upto 6 months post-op. within 3 months, probably i would think the wound inside is healing and hence you may see swellings..
Is there any concrete reason for swelling especially more than 3 months post-op?
can it due to some physical excersises or any other activity which puts a lot of strain in the operated area?
