Had surgery with Fielding March 12th.

Is there any benefit to wearing the vest longer than the 3 weeks he suggests (I'm three weeks tomorrow)?

Keep in mind, I accidently fell asleep a couple days ago without it on and woke up feeling bad about it.

I mean, I know there's no downside to wearing the vest say for another week, but if Im worried about potential nipple puffyness reoccuring- do you think it helps? Or is it really not necessary?

I generally recommend that my patients wear a very snug compression garment for at least 3 weeks post op.  I know that wearing it is a real pain in the A--!  But actually a good, firm garment is extremely important to the healing process.  It will minimize swelling, bruising, and fluid accumulation and actually allow for faster healing. 

Interestingly, many of my patients after a while really feel better (and more "secure") while wearing their garment. And wearing it after the 3 week period can only help -- it cannot hurt at all.

If you wish, you can switch to another type of firm garment, such as recommended on this site, for a month or more after the initial 3-4 weeks of tight compression.

I find Underarmour shirts a bit skimpy.  If you want to use Underarmour, but something at least one or two sizes too small for you -- and then wear two at a time!

I can't tell simply from a photo -- this is something that you have to feel in your hand and observe on a person in order to assess it properly.
