Directions to fieldings office

Hi can anyone please give me directions to Dr. Fieldings office? I am taking the subway.

Get off at Jane Station and proceed to the exits.  As you come out make a left so you are now walking toward Bloor Street.  The building is called 'XXXXX (can't recall) Medical Centre' and is near the traffic lights.  There's a Coffee Time directly opposite it as well.  

I believe JCF's office is on the 4th floor of the building.  You can look up the building directory to be safe though.  The building is actually built like an apartment/condo so I was knocking on his door thinking that he would open it.
