I have been browsing the beach body forums and have found some inspiration for increasing your fitness before thinking about surgery. Take a look at these threads, and their before and after photos. I have also linked to a site called bodiesbymike.com. I emailed the guy after being amazed by his before and after pics, and he emailed back, which is down below. He had puffy nipples yet managed to
firm them up.
Here are links to threads in the "power 90 photo gallery" with some people who have breast development and or puffy nipples who managed to make them look amazing.
My new mindset is, until you can see your six abs without flexing, don't get the surgery.

Thanks for the email. The puffyness around the nipple area was mostly
fat and water retention. Most guys get it as their bodyfat levels get above 15%-20%. I was doing the same basic exercises I had been doing all along for the most part. The diet was what got rid of the puffyness and allowed
my
natural shape to come out. As far as what exercises I do for chest. My
routine almost always had flat bench and 45 degree dumbbell incline
presses
in it. From time to time I would cycle in dips, cable crossovers, smith
incline, hammer incline, and various other chest press machines along
with
different styles of pushups etc. However, my mainstays have always been
flat
bench and dumbbell incline presses. Most of my workouts were geared
toward
working up to a heavy set of 5 on the bench and a heavy set of 10-15 on
the
db incline.
I would also like to add that the shape of the pecs is pretty much
genetic.
Over the years I have actually tried to get more of a "squared off"
look to
the pecs like Steve Reeves had but my natural shape never changed. I
could
get them bigger but the shape remained the same.

I noticed on your before and after photos that your
chest looked 100% better than that side chest shot.
Could you tell me specifically what you did to reduce
the puffyness around the nipple area? The chest looks
perfect in the after shot by the way.
