Post op Scar ulcer after four months

Docs please help I'm currently travelling around asia with a heavy back pack in extreme heat and sun exposure , im using sun block on my nipple scars but i have developed an ulcer on one of my scars that was previously healed , i dont think that its a spitting stitch as i cant feel anything. Im from the UK and wont be back to my PS for 7 months ,  , what would your advice be really worried it might be necrosis as feeling never returned to the nipple but it looks healthy and pink. Should i just keep it clean and use a steristrip ??
