Gynecomastia is a contour problem. If there are female like breasts on the chest then it is gynecomastia by definition. If you are asking if you can differentiate gland from fat, all male mammals will have a gland component, it is part of being mammal. Male Chest Exam alone is not sufficient to distinguish fat from gland. Fat tends to be soft and gland firm, but fat can be firm and gland soft.

One picture alone is not enough for a determination, but if there are global fat issues such as what seems to be demonstrated in the single image, then weight loss first is often the best first choice. Weight Loss can help with the fat component of gynecomastia. Gland, localized fat, and skin can remain as contour issues.
