scar lump 18mo after surgery

I had surgery November 2010 as a redo since the original surgery was too cautious and there was little change.  This time I had a large mass afterwards under the nipple and was told it is scar tissue and will go away.  I came back to the doctor months later and was told to wait a year and then two years.  It is now 18 months and it has gone down but it still hard, about 3/4 inch diameter and the nipple around is puffy, several times bigger than the other one.  Is this something that a steroid shot can help, doctor did not return my last inquiry and I am not wild on going back to his office.  Thanks.
