what size macom vest to order?

i had surgery done 5 days ago,

my chest measures 34 inches, but i am confused what size macom vest to buy. my surgeon told me to buy a size down from 34 inches/.

the sizes on the website are a bit confusing however. it says that a small size is >36 inches. but it doesnt say anything about the extra small size. so should i be buying an extra small as i am 34 inches?
