Would love your advice/opinions, I'm 5'9 ~165 pounds and I've noticed, for sometime now, that my nipples are quite puffy. Is it fat or I have gyno?

Gynecomastia is essentially any breast growth (whether fat or glandular tissue or a combination) so based on that definition I would say yes you do have it but it's always difficult to gauge severity/grade from photos taken at awkward angles...best thing to do is see a doctor who will be able examine you.

He his right! Gynecomastia is the word that describes male breast enlargement! So, if your a male and you have enlarged breast, then you have gynecomastia, and it does not matter if it has more fat then tissue or more tissue then then fat!

I'm sure that you know this, but I'll say it anyway. We are all born with breast tissue and a males breast is really no different then a females breast.

Surgery is a personal choice! I'm 56 and have grown to 46H size breast due to health problems. I don't fear surgery at all, in fact I may need my third back surgery soon, and it will be my 8th and two where total knee replacement surgery.

If you are really bothered my your breast and have the money to spend go for it! I had gyne all my life, but times were different when I was your age and we didn't make a big deal out of men with bigger chest. So do what ever you must do.

Losing weight before surgery would be ideal for the final result, although i think you are in decent shape for surgery right now. To answer your question if i was in your shoes i would do the surgery. Its all up to you though

Surgery is a very personal and often costly decision to make. Strictly speaking, no one really needs surgery. You only need surgery if you have some sort of pathology and that is rare. Surgery is usually elective.  That is to say that there is not a medical need for it.  Surgery is the only way to reduce the breasts.  There are a number of companies that will take your money for bogus cures, but they will do nothing effectively but reduce your wallet.

If surgery is still an interest, There are a large number of unqualified people willing to cut on you for a handsome price.  To be safe use only a surgeon who is qualified and who specializes in this kind of surgery. If you are going to lose some weight, it is best to lose the weight prior to surgery.
