I understand that in a lot of surgeries the incisions are made around the lower half of the nipples. If they're surrounded with hair does this damage any hair follicles?

The peri-areolar incision is made just at the dividing line between the pigmented areolar skin and normal skin.  Or, it can be made 1-2 mm within the areolar skin (my preference).  In either situation, there are no hair follicles present.
