It's exactly 4 weeks post op and for the past week the vest has not been nearly as tight as it was in the beginning. Does it matter at this point? I've been instructed to wear for 2 more weeks.

It may be that the vest is just as tight -- but your swelling has reduced (as expected) -- so the vest feels somewhat looser.  Try to wear the vest at the tightest setting (if it is adjustable).  And good idea to wear it as long as possible.  I recommend six weeks at a minimum for my patients.

I wear a tight tshirt underneath it now. I should have thought of this right away, neglecting it for a while doesn't harm the final result does it?

The vests I use do get a bit looser with time . Around a month for the ones I use is typical.Combine the reduction in elasticity with the decrease in swelling as mentioned and the fit is not as tight.
