I'm posting here along with the Ask a Doctor forum.
I am one year post op and one of my nipples points downward if I make use of my chest muscle in any way.

I'm curious if anybody has seen anything like this before..?

Absent a physical exam to confirm, it appears that there is residual tissue under the nipple and just above, which is tilting the nipple downwards.  That tissue may be residual gland or new scar tissue.  Best advice is to return to your surgeon to discuss what to do about the result.

One quick question...
My doctor is telling me that I have to pay to have this corrected by him. Is that common?

Difficult to answer your question.  Realize, it is one year since your surgery.  If you were my patient, I would offer re-do surgery at minimal cost -- simply for anesthesia and operating room costs.  This is something you should discuss with your own surgeon however.  There are no hard and fast rules on this unfortunately.
