On the disability front … not all disabilities are visible . I have profound bilateral hearing loss and function okay with the advanced hearing aid systems of today. But I still get easily spooked in parking lots and walking on the streets.

I am going to try an extender and see if everything stays in place.  If somebody wants to get into the men's market, we need larger bands with smaller cups.  I am a little overweight, but I am not what anybody would consider a large man.
In my search for a brassiere that fits I struggled with this issue... large band but smaller cups.  The solution I found is the one you are considering... buying a brassiere with a smaller band and using both an extender and a strap that holds the shoulder straps in place.  I did find a 44B brassiere that doesn't required an extender and bought it in three different colors.  I'm wearing the nude brassiere as I write this.  Unfortunately, the supporting frame is a bit heavy and becomes uncomfortable with long wear.  I put it on about four hours ago and will likely take it off soon.  The brassiere that has worked for me is a 42C which I can wear all day with the extender and strap to hold the shoulder straps in place.  I've bought perhaps tens of those brassieres in different colors, five of which are set aside for future wear.  Sadly the manufacturer no longer makes the 42C.  At least for now I'm not concerned but it is pretty clear to me that to get a brassiere that fits... an unpadded, underwire brassiere, I'll need to got for a narrower band and an extender.  Good luck sorting this out.

I am currently wearing soft cup 42B underwires and I can wear some of my wife's 42Cs.  According to her, I should probably be in a 44B.  What she did not touch on was that, once you get past a 40 band, smaller cups are non existent, simply because women with large chests tend to have large breasts and there is no market for them, except for us.  I have a Bali Lace Desire in a 42B, but they only come in black because they don't expect to sell a lot of them.  Ideally, I should probably be in a 44B but I don't think anybody makes them in a soft cup.  I am going to try an extender and see if everything stays in place.  If somebody wants to get into the men's market, we need larger bands with smaller cups.  I am a little overweight, but I am not what anybody would consider a large man.
I have some suggestions for you.

There is an online store called Her Room which has more than 2200 individual models of bra available. I sometimes browse there, and what makes it appealing is the very detailed search filters available. It also has a comprehensive analysis for each bra, including things like strap width, gore height, hook count etc. For fun, I dialed in 44B, underwire, unlined, and got 22 hits. Here is the link:

Ignoring the very expensive bras in the results, and since you are already familiar with Bali bras, may I suggest their Flower bra? Note that prices are in AU$, unless it automatically defaults to your local currency.

What a great video. I am 79 yo male and have developed through medications and an implantable pacemaker.  As a result my left breast is larger than my right. I am not large 38b but find a good bra not only supports my breast but also protects and supports my pacemaker.  I find a crop top is useful if wearing a light shirt as it is virtually invisible but the support is average. I normally wear a underwire full cover or a soft cup full cover. A lot of trial and error to find the best fit. I tend to wear 24/7 mainly for the support in the pacemaker area. This is a great site and has helped me in not only bra selection but acceptance as well.
