if an underlying cause is addressed, is there a chance it will go away on its own?  Or is surgery still the only option?

i know for a fact, that if u have puffy nipples, the underlying issue is a gland behind them and the only solution is to not let other peoples thoughts and comments bother you or get surgery.

There are two important factors which need to be taken into account: Age and Heredity.

Your outcome will likely be similar to your father and uncles.  If they, as adults, still have the condition: you will likely have it as an adult also. If they had Gynecomastia while in their teens and the condition resolved on its own, You could probably look forward to the same happy result.

Age is a bit more complex. If you have had the condition for several years with no significant change, surgery will  be your only sure way to get rid of it.
