Is it Just Fat? or Gyne?

puffy nipple poking out has been stressing me out years and years.
I want to know whether i can get rid of this by exercise or there must be surgical excision and lipo done on this.

You're not overly fat and you have a well defined waist. In my opinion it's clearly gynecomastia.

Who should I be going to check my condition? family doctor  or cosmetic physician?

Yes, you have gyne and you would probably require surgery to remove it completely.  Suggest you visit a gyne expert in your area for an opinion -- or consider traveling to see one.
