I have posted a topic regarding it before and saying that i got it at the age of 14 and im currently 17 and doctor said that it should go 95% of times in around 3 years , if it was to go how will it go. will the gland size diminish or spread to reduce the puffiness of the nipple ?

The gland involutes over time if it is going to go away.  Another example of this is in women who have babies and breast feed.  During the pregnancy and post-partum periods, the breasts are often large and engorged with breast milk.  Once breast feeding is over, the breasts shrink in size and the glandular tissue dissipates.  This is on a much bigger scale than most patients with gynecomastia, but it demonstrates how the tissue just sort of goes away.

Is it a slow process to be completed like for the gland to involute or does it happen quite quickly ?

It takes place over time. It's not overnight.
