Title: Why is there "Dr. Removed" ? ? ? ? ?

I've been doing a little research on which doctor I'm considering for gyno surgery and it appears that there is a doctor that was once popular here who is now labeled as a "Dr. Removed" title.

Why is this?

I think everyone knows who I'm talking about. If not, then read this backwards...

Can someone point me in the right direction...
