What's the chances of the nipple caving in if all the gland tissue is removed ?

100% if all the tissue is removed. It must be remembered that all of us have some breast tissue. The breasts of all children are the same regardless of gender.
One reason for getting the services of a Cosmetic Surgeon instead of a General Surgeon Is that they know when to stop. Crater deformities are much more common with General Surgeons.

When I perform gyne surgery, I am very careful about how much tissue I remove because I want to avoid a cave-in (we call it a crater deformity) and I am acutely aware of this during surgery. If I feel there is any contour irregularity during surgery, then I immediately take appropriate steps during surgery to correct the problem. That is the advantage of going to an expert in gyne surgery.
If there is a contour irregularity (crater) of the nipples after surgery, it will not self-correct -- you would require revision surgery.

The more I read about surgeons discussing gynecomastia surgery, the more I see how incredibly diffult and unpredictable it is. Especially for the more complicated cases where both lipo and gland excision is required and also has to deal with nipple modification.

All surgery is difficult and unpredictable. That is why this forum is so important.
If you get the gist, surgeons who have done thousands of these procedures have large experience in performing the procedure and can usually avoid many of the problems that are encountered with less experienced surgeons. There are no easy gyne surgeries -- even the ones that look like a slam dunk. They can all be screwed up!!
Unfortunately, gyne experts are scattered throughout the country -- and sometimes it pays to travel for such expertise. Understand price is a consideration for many -- but at what cost?? Poor results, disappointment and possible need for revision surgery at additional cost.
These discussions are not meant to scare someone out of surgery -- but rather to educate guys to do their homework and go to a real expert in your area.
