Title: Do I need an excision or just liposuction (gynecomastia)

Hi I have rise in the chest area and as especially behind the nipple and the region is a little bit hard and firm even when I nearly loose 100lb the chest area change just a little so I asked my surgeon if I need a excision and he told me that I don't need excision I just need liposuction so my question from the picture do you think I need excision or just liposuction..this is the photo..http://d.top4top.net/p_237kie50.png....

Nice job in losing the weight.  You most likely have a gland component to your chest unless you have psuedogynecomastia which is fat only.  It is difficult to say without an exam but my feeling is that liposuction alone would be incomplete.

You really need an exam. If there is a hard, dense area of thickening beneath the nipple, direct excision is probably required in addition to the lipo.

Congratulations on your weight loss. You look great! From what you are describing, I would be inclined to say excision of the glandular tissue would be necessary. An in-peson examination is really necessary to determine the components of your gynecomastia (fatty vs glandular or both). I rarely perform liposuction only, because it doesn't usually achieve the best result. 
I would recommend getting more than one opinion. Be sure you are seeking out the advice from board-certified plastic surgeons who truly specialist in male breast reduction surgery.

Congratulations on your weight loss.  To answer your question, I use a combination of liposuction and excision in nearly all cases in order to provide complete treatment.  Liposuction alone may not be adequate to eliminate any glandular component, and this would be more prominent after suctioning of the surrounding fat.  You should consult with a surgeon who does gynecomastia surgery on a regular basis.
