Surgeons that reinforce the nipple with fat graft

Who are the surgeons that remove the entire gland (so it will never become puffy again) and reinforce the nipple with a fat graft to stop it from caving? I heard Dr. Jacobs is one of them. Who else?


It seems most of the surgeons leave gland behind the nipple to stop caving but by doing that there is always the risk of recurrence of puffy nipples. 
Thoughts?
