revision needed after poland op & botox shots

My puffy nipples are still there.  Gyne is present in my chest.  I had surgery in poland.   It was glad removal, not lipo.  Doc said i'm thin and fit so only gland needed.  max 20% improvement I have had.  Its been years already.  I went to the doc for revision, he said skin is loose so another major surgery would be needed to tighten it (as he says gland tissue is gone) {yet gene puffy nipples are embarrassingly apparent}.  For the time being, he injected botox into my nipples.  For about 5 months I had some improvement.  But thats it.  I want a revision, but not going back to same doc.  And another surgeon I contacted said they won't do a revision because some other surgeon did it first.
Please help.. what shall I do?
