difference in puffy nipples

i was wandering if anyone could explain the difference between puffy nipple and full blown gyno? i definitely have some sort of fat or gland there but when nipples are cold they look normal and chest looks fine, is this puffy nipples then? im having a consultation soon so will find out from a surgeon but would be nice to have the knowledge before hand. please reply
