18 stone, 6.3 f.t tall, 19 years old.

Is this Gynecosmastia or fat?

Both.  You have gynecomastia and your obesity makes things look even worse in the chest area.  If you were to lose weight, however, the look of your chest would probably not improve that much. When we lose weight, the fat in our chest area is the last to go. 

Most doctors would tell you to lose weight and aim for your recommended weight range prior to looking into surgery for your gyne.

not to be mean but why would u even worry about having gyne or not? should worry about your weight most importantly.. lose some fat then see if u have gyne or not

I'm no doctor but I'd say that is definitely gyne if I were a betting man....due to intend to have surgery or just content with it as is?

Yes, it is gynecomastia.  Obvious.  Just take the pencil test.  8) 

If you lose some weight, you might also find that you do not lose much in the breasts.
