help with light gyne

Hi guys.
I am 29yo with a normal physique.
When younger I was pretty fat and had issues with gynecomastia. I was really bullied for it and had a terrible experience.
Time went by, fat melted away, I started working out and things changed.
As an adult I relaxed over time but on the last 3 years I took some measures and got back in shape again. I managed to get super lean with 90kg on the scale and only10% bf. That's the leanest I have been, but even at that level of muscle pump I had that puffy nipple that would
Haunt me from early days on.

I am now much lighter, 80kg and 15% bf.
I know I have some fat up there but what pisses me off is this puffy nipples.
It is driving me crazy, I keep touching it to make it flex and look hard in order to hide it. But it is a nightmare .

I know there are much more severe cases but I would like to know you guys opinion.

I have visited a surgeon here in Brazil and he recommended the surgery taking the glands
Out plus liposuction , but he tells me that he must leave some gland in order to prevent depression, but I am worried he is more concerned about the lipo instead
Of the gland.

Anyway, I am visiting another doctor next week to evaluate my feelings.

Definitely puffy nipples and mild gyne.  Your surgeon's operative plan seems to be appropriate.
