im wondering if diet and exercise will lose weight in my chest area i have tried before in my past and cant seem to get results in the chest area.. is surgery the only option? what are my realistic expectations with surgery? can i expect a flat chest?

dude no offense but your living the hell out of those stretch marks.. it's all over your body.. I'd suggest to work out not because it could remove your gyno but for you to enjoy life and be healthy atleast. Regarding your chest? that's a severe case.

Have you checked your Body Mass Index with a BMI Calculator?  BMI does not differentiate between fat, muscle, and bone.  Body Fat Analyzers can help with the fat percentage and are better at helping define the fat component.

Plastic Surgery is not an alternative to losing weight.  With surgery, a fat person will still look like a fat person, just one with smaller breasts.

The problem is that Male Fat Pattern extends around the chest, under the arms, and around the back.  Plastic Surgery is not a good tool for a global fat problem.

I advise my patients to get to a weight / body fat percentage they are comfortable with before considering surgery. 
 
Weight Loss Before Gynecomastia Surgery can help with the fat, but not the gland.  However, you cannot pick where your fat comes from. Plastic Surgery is also not a good jump start tool for weight loss.  I have seen disasters from patients from other doctors with deformities from significant weight loss after their surgery.  Men tend to put fat on first in the belly and chest bands.  We tend to take of those areas last.  Early surgery and depending on weight loss to predictably change the body is a nasty gamble.  No Surgery Body Shaping Garments are a better temporizing choice.

As a surgical sculptor, I view weight loss as a coarse tool and my plastic surgery as a refinement tool.  I prefer to use the coarse tool first, and then my surgical sculpture for refinement.

You heard it from the doc.

If you want to change the shape of your body you're going to have to work at it. You need to adjust your diet and increase your exercise. Only then when you're at a healthy weight should you consider surgery.

Time for a lifestyle change bro.

I see imageshack have removed your photos due to "violation" You really MUST challenge this! Ive seen this before , they class your pix as "soft porn" ie bare female breasts. You cannot allow this to happen, it is  illegal to discriminate against person with disabilities or disfigurement. you can easily argue that gynecomastia is such. Please do not allow them to get away with this disgraceful behaviour! >:( >:( >:(

I don't mind looking like a fat person with no breasts though like if I could just have stomach and no breast I would b more satisfied is there a option to just remove chest fat/ gland? Flat chest big belly?

I didn't remove the pics i guess imageshack did I understand what you saying but I came here to ask about the possiblity of surgical options I'm 6'5 360 I don't mind being a big guy I just want to be a big guy with a flat chest

It cannot be used as an alternative to weight loss. Plastic Surgery can be used to make very fat peoples breasts smaller, but there will still be breasts. Plastic surgery is best for recontouring the body, not for general weight loss.  Most of the time, fat is located where plastic surgery is not best directed (such as the apron of fat BEHIND the abdominal wall).  We are able to help sculpt loose sagging tissues after weight loss:

Morbid Obesity is dangerous to health, that is why it is called morbid in the first place. Elective surgery on Morbidly Obese patients is particularly risky and carries an unreasonable complication rate.
