Lose overall body fat % less puffy nipples?

If you read some of my older topics you'll see that I am concerned about my puffy nipples, I have been accepted to go back to my surgeon for a revision but I am unsure if this is the best option.

My nipples look amazing when my nipples are erect but when soft not they are puffy.

Right under my nipples there is nothing hard just some soft fat, could this be causing the puffiness?

When I bring my arms all the way up my nipples go into my chest which makes me think there is no breast tissue there?
