how do people pay for surgery ? or are loans easy to obtain with no credit

Unfortunately, if you have no credit (or bad credit) or no credit history, banks will not be eager to provide you with a loan.  You may be able to have someone with a positive credit history co-sign the loan for you -- but then they will be responsible (and quite ticked off) if you should default.

Best is work hard, save some money, and perhaps add to it with small loans from relatives or from a bank.
