Hi Doctors,

My name is Ali and I used have severe gynecomastia. I had my first surgery a year ago and then 2 revision surgeries after. As you can see from the photos below, I no longer have any  gynecomastia tissue left. However, the scarring on my areola looks terrible and noticeable.

My surgeon says that my chest look great compared to what it used to look like. I do agree with him in terms of looking fine in clothing, but as soon as I'm topless It looks wrong. I really don't want people to notice that I had done gynecomastia surgery.

So my question is: is it in any way possible to fix the scars around my areola? or at least make them unnoticeable?

Your surgeon probably performed a "donut mastopexy" by removing skin around the edges of the nipple and then drawing the outer skin centrally to meet the nipples.  This usually results in scars completely (360 degrees)around the edge of the areola. I have never been a proponent of this type of surgery because of the scars and potential areolar deformities that might occur -- and you have poor scars and irregular contours and stretching of the areolas -- just as would be predicted.

Unfortunately, once this is done, all the bridges have been burned and there is no treatment to remove the scars.  If the scars are thickened, then perhaps some careful injections of cortisone (like kenalog) could help.  Cortisone can flatten the scar but it will allow the scar to spread out a bit (sort of like melting jello).  In time the scars should fade somewhat.

Surgery is a matter of constantly balancing the risks versus the benefits. Skin and / or areolar tissue can be removed however the cost is scaring. The scaring needs to be balanced against the increased improvement from the tissue resection.Once the tissue is resected and the scar is in place as Dr Jacobs implied the cat is out of the proverbial bag.
