Only now catching up to this post, and Dr. Jacobs has very nicely addressed your concerns.  One point I'd make about crescent excision of skin--while many areolae are not perfectly round (most seem to be slightly oval shaped in repose), the crescent excision will certainly result in a slight change in shape, depending on how much is excised.  If you're undergoing fat flaps, I'd probably wait until after that revision to do any skin excision, particularly since skin excision can be done very easily with straight local anesthesia.

Thank you for your feedback, Dr. Silverman. Any thoughts on whether the revision surgeon removed too much tissue or whether the creases, folds and indents are purely due to skin adhesion?
