Recommendations for surgeon in TAS? Or mainland doc who does full removal?

I've seen 4 plastic surgeons in Hobart. They have all said they don't do full gland removal. In fact one said it is impossible and others said the nipple will die from gangrene without part of the tissue left behind the areola. They said it would also increase the chances of concave nipple.

I'm on TRT so it is important to me to have the entire thing removed so I don't have to worry about it coming back. 

Can anyone recommend a surgeon in Tasmania? Alternatively, any recommendations for a surgeon  on the mainland? I'm so far unconvinced about Dr Lanzer based on some mixed reviews I've read. Its a lot of money, and I don't like the idea of having to travel for pre/post OP appointments, travelling while recovering etc. - unless I know it's going to be worth it.
