Dr Jacobs nyc suggests trying lipo before excision

I think I'm going to get surgery with Dr Jacobs in New york sometime this year. When i had my consultation he said he will try just lipo first, and then if that doesn't work (if the gland tissue is too hard) then he will decide mid surgery to do excision.

I seem to be reading that most people are not happy with lipo alone and I don't want ot go through all this and not be happy in the end. Should I insist on him doing excision also? Has anyone had Dr. Jacobs do lipo only and be happy?

Why do some of these guys try to go with lipo only when it's clearly not satisfactory most of the time?  Excision requires more skill, maybe?  Makes me wonder about their level of expertise.

my doctor said if gland is removed when it doesnt need to be, it can result in a caved in pec.
