Have there ever been cases where teens lose glandular gynecomastia during puberty???   I thought the chances were 90 %?

Yes but I suspect the rate of 90% is a gross exageration.

I could accept the word usually, if it was understood that it meant something more than half of the time.

The glandular growth had a cause, and usually that cause was puberty.  When the hormones get straightened out, The glandular tissue will stop growing and should slowly shrink.  The key word is slowly.

For some of us, Gynecomastia is a family trait.  This means that if you know the history of your Father and uncles, It should be a fairly reliable predictor of what you will experience.  In my family all the men have small but well defined breasts.  For us it never did diminish. Hopefully you are more fortunate.
