Each company has its own policy, and they vary widely.

Some have been forced to pay for surgery after being sued;  this has loosened things up a bit.  

In some cases, a medical need must be demonstrated.  If your company is among the latter,  A letter from a Psychologist Can be helpful.  Some young men with Gynecomastia have a low sense of self worth, problems socializing etc.  A Psychologist would be of help here in indicating that the surgery would help with those issues.

First, just ask and see if they will cover the surgery.  If not, then see a psychologist and file an appeal.
