I had surgery about 10 weeks ago and still have hardness under each nipple. The hardness seems to soften when I'm in the shower. Is it scar tissue or left over from the previous lumps before surgery?

If your nipples were initially flat and flush with the surrounding skin after surgery and are now puffing out a bit, it sounds like this may be scar tissue.  Scar tissue usually forms between 4-12 weeks after surgery.  You should also inquire of your  surgeon as to how much tissue was removed from the area just under the nipples.

If indeed it is scar tissue, then a visit to your surgeon is in order to determine if treatment or observation is the best way to go.
